INSTRUCTIONS

EES Authorizing Contact Designation/Agreement (Form 6504)

The person who signs the EES Employer Confidentiality Agreement (Form 6503) also signs the EES Authorizing Contact Designation/Agreement (Form 6504). The EES Authorizing Contact Designation/Agreement (Form 6504) is used to designate one individual at your organization who will be responsible for granting permission to the Retirement Systems to issue USERIDs to your designated agents. Be sure to submit a new EES Authorizing Contact Designation/Agreement (Form 6504) if your authorizing contact changes. Since the authorizing contact must approve who gets access to EES, the authorizing contact should be someone in your organization who knows when it may become necessary to terminate or initiate an agent's authorization so that agents' access to EES can be canceled or initiated as necessary.
