EES Designated Agent Confidentiality Agreement (Form 6505)

 

You must complete a separate EES Designated Agent Confidentiality Agreement (Form 6505) for each Agent for whom you are requesting access. Both the Agent for whom access is being requested and the Authorizing Contact must complete, sign, and date the form. Please provide an email address for the Agent. This allows PEBA to notify all EES users by email of changes to the system or its status. It is important to notify PEBA if an Agent's EES access needs to be terminated because anyone with a USERID and a PASSWORD can access EES. The Authorizing Contact may do this when submitting a new EES Designated Agent Confidentiality Agreement (Form 6505) by writing across the top of the form: PLEASE TERMINATE ACCESS FOR JOHN DOE. The Authorizing Contact may also terminate access by sending a letter to the address on the first page of this package or an email to the EES Support Desk at eessupport@peba.sc.gov.

 

Within one week of receipt and processing of an EES Designated Agent Confidentiality Agreement, the Agent will receive an email from PEBA which provides the EES Internet address, Agent's USERID, and PASSWORD. If an Agent loses or forgets their PASSWORD, send an email to the EES Support Desk at eessupport@peba.sc.gov that contains the Agent's name and USER ID. PEBA will issue a new PASSWORD by sending the Agent an email containing the new PASSWORD.

Member Inquiry:  User will be able to view member data and create benefit estimates.


Member Maintenance:

Enrollment:  User will be able to submit and maintain online enrollments.

Return to Work: User will be able to submit Return to Work transactions.

NOTE:  Member Maintenance is not applicable to employers who use SCEIS.


Employer Reporting:  User will be able to view deposit/quarterly reporting status and transaction error list; create deposit forms, submit quarterly reporting and furlough data, and submit contribution payments electronically.

Final Payroll Certification: User will be able to certify final pay for service and disability retirement and information needed to pay retirement benefits.   

Claims Financial Transactions: User will be able to certify termination date and final retirement deductions for refunds (Form 4201).

Claims HR Transactions: User will be able to certify last day paid (Form 6201) for service and disability retirement and information needed to pay death claims.
